Initial Visit & Insurance Information
WHAT TO BRING
Please bring your insurance card(s) with you.
We will make a copy for your file. As a convenience we will bill
most of the major insurance providers. Your insurance plan my
require an insurance referral. (This is not the same as a medical
referral) Depending on your plan you may need a referral for each
visit. If a referral is needed you must bring this with you at the
time of your appointment. You will need to fill out a Patient
Information Form before your initial appointment. You can

For Workers Compensation or Auto Claims please bring the name and address of your employers workers compensation /auto company, the claim number and your date of injury. Your claim representative’s name and phone number are also helpful.

A signed release allowing information sent to us.

The reports or actual films for any X-ray, CT, MRI or other tests pertinent to your problem.

Copy of any lab work (blood tests etc.) that have been done in the past 6 months.

Any pertinent hospital or physician notes.

List of all your medications and dosage.
